Intismeran (full name: intismeran autogene, also known as V940 or mRNA-4157) is a personalized mRNA cancer vaccine from Moderna and Merck. It is an individualized neoantigen therapy. Doctors sequence a patient’s tumor and normal DNA to identify mutations unique to that person’s cancer. An algorithm then selects up to 34 of those neoantigens and encodes them into a single synthetic mRNA molecule. Once injected, the mRNA instructs the patient’s cells to produce those cancer-specific proteins, training the immune system to recognize and attack the tumor cells. It is given together with Merck’s Keytruda (pembrolizumab), a PD-1 checkpoint inhibitor that helps keep the immune response active. The combination is currently being studied mainly in the adjuvant setting (after surgery) for high-risk melanoma, with additional trials in lung, bladder, and kidney cancers. In the Phase 3 INTerpath-001 trial in completely resected stage IIB–IV melanoma, intismeran + Keytruda reduced the risk of recurrence or death by 42% versus Keytruda alone.
